    The effect of language knowledge on speech perception: what are we really assessing?

    Objective: The authors examined whether open-set speech perception scores are limited by knowledge of vocabulary and syntax and further considered whether remediation of vocabulary and syntax will increase open-set speech perception scores. Study Design: This was a repeated-measures study design in the setting of a primary (elementary) school for the hearing impaired. Patients: The study population was composed of three hearing-impaired children using Nucleus 22-channel cochlear implant. Intervention: Intervention used was language remediation sessions. Main Outcome Measures: The main outcome measures were assessment of auditory-alone speech perception benefit using open-set words and sentences and assessment of syntactic knowledge using the Test of Syntactic Ability. Outcome measures were applied before and after remediation. Results: Child 1 and child 2 showed a significant postremediation improvement in their overall scores on the Test of Syntactic Ability and in their ability to perceive words learned during remediation. Child I and child 2 also showed a significant improvement in their scores on a modified Bamford-Kowal-Bench open-set sentence test, which specifically targeted grammatical constructs trained in remediation sessions. Conclusions: Remediation of language knowledge deficits significantly improved open-set speech perception for two children, suggesting a need to include language remediation in cochlear implant habilitation programs.

    The effect of loudness imbalance between electrodes in cochlear implant users

    Objective: The aim was to determine the effect of loudness imbalance between electrodes in patients using the 22-electrode cochlear implant (Cochlear Pty Ltd). It was hypothesized that speech perception scores would be greater when the loudness of electrodes was balanced at the comfort (C) levels than when the C levels were unbalanced. Design: Ten adult patients received a monosyllabic word test (CNC words) in quiet and a sentence test (CUNY sentences) in noise under two conditions: with C levels balanced for equal loudness and with unbalanced C levels. Results: When the C levels across electrodes were pseudo-randomly unbalanced by 0 to ±20% of the electrodes' dynamic ranges (20% unbalancing), 6 of the 10 subjects showed a significant drop in sentence perception scores. Of these patients, none had a significant decrease in perception when the degree of unbalancing was halved. Of the four patients who showed no change with 20% unbalancing, three revealed a significant decline in sentence perception when the degree of unbalancing was doubled. There also were significant group effects for phonemes on the word test as well as for sentences in noise for the 20% unbalancing. Conclusions: The implications for clinical practice are that it is important to balance the C levels and that clinicians should be encouraged to refine methods for setting C levels in very young children, who may be using unbalanced MAPs. Nevertheless, although most patients revealed a statistically significant drop in sentence perception with 20% imbalance of the C levels, the changes in percentage scores often were only small.

    Prediction of variance in neural response to cochlear implant stimulation and its implications for perception [Abstract]

    Cochlear implant patients' perception of sound is derived via electrical pulses arising from an electrode array. Chosen aspects of the acoustic spectrum are coded via a stimulation pattern designed according to some sound coding algorithm. Thus, a patients' ability to discriminate between sounds, and in turn their understanding, is directly related to their ability to differentiate between the patterns of electrical stimulation which code various sounds.

    Current speech perception benefits for adults using the speak strategy with the nucleus 22-channel cochlear implant

    Present day recipients of the Nucleus 22-channel cochlear implant make use of the SPEAK speech processing strategy in the Spectra 22 speech processor. Introduced in 1994, the SPEAK strategy is based upon research conducted at the University of Melbourne and the Bionic Ear Institute. This paper compares the pre-and post-operative speech perception abilities of adults who have used the SPEAK speech processing strategy since the 'start-up' of their cochlear implant system. Data was analysed from open-set sentence and word tests administered pre-operatively and at regular intervals postoperatively. Patients showed significant improvement in their ability to recognise speech, when listening with the cochlear implant without lipreading, within the first two weeks. Mean scores for open-set sentences were 15% preoperatively and 64% two weeks after initial programming.

    Speech cues for cochlear implantees: spectral discrimination

    The relationship between the ability of cochlear implantees to perceive speech and their ability to discriminate between stimulation on adjacent electrodes was investigated. Speech perception ability was assessed with monosyllabic words in 8 users Nucleus cochlear prosthesis. The ability of these subjects to discriminate between stimulation on adjacent electrodes, in of random loudness differences between electrodes was determined. Results were averaged in the apical, mid and basal regions of the cochlea. Regression analysis showed that speech perception ability and electrode discrimination ability were correlated in the apical to mid region of the cochlea. but the two factors were not correlated in the basal region. Therefore, these results suggest that implantees require the ability to discriminate between stimulation on adjacent electrodes in the apical to mid region of the cochlea in order to achieve high speech perception scores.

    Speech cues for cochlear implantees: spectral discrimination [Abstract]

    The ability of cochlear implantees to understand speech varies over a wide range. While some implantees achieve scores close to 100 % open set word tests, other require visual cues to achieve a significant score on these tests. The focus of this research is to investigate reasons for the wide range of ability and therefore to improve the speech processors used by individual implantees. This study first investigated whether the relative importance of various frequency regions of the speech spectrum differs for implantees of different performance levels, and for implantee groups compared to normally hearing subjects.

    Speech perception of hearing aid users versus cochlear implantees [Abstract]

    Recent improvements in speech recognition for profoundly deaf, cochlear implant patients have suggested that some people with a severe hearing impairment would be more successful with a cochlear implant than a hearing aid. Unfortunately, no studies have investigated the speech perception of the severely hearing impaired in order compare their speech perception abilities with those of cochlear implant users. To investigate this area further, a detailed investigation of the hearing aid performance of people with a severe hearing loss is being conducted in the Department of Otolaryngology at The University of Melbourne. The range of hearing losses for this study was defined as a pure tone average of greater than 6OdBHL, but no worse than 100dBSPL in the better ear. At present thirty-five (n=35) participants have been involved in this study. Each participant took part in a series of speech perception tests which included 24 consonant recognition, 11 vowel recognition, CNC words, CUNY sentences, and the connected speech test.

    Speech perception of hearing aid users versus cochlear implantees

    Recent improvements in speech recognition abilities for profoundly deaf cochlear implantees suggests that some adults with a severe hearing impairment might benefit from a cochlear implant. Unfortunately, no studies have directly compared the speech perception abilities of the severely hearing impaired with those of cochlear implant users. An investigation of the speech perception performance of people with a severe hearing loss, who use hearing aids, was conducted in the Department of Otolaryngology at The University of Melbourne. Each participant (n=35) took part in a series of speech perception tests which included 11 vowel recognition tests, CNC words, CUNY sentences, and the Connected Speech Test. The results from these severely hearing impaired persons were compared to results from cochlear implant users. The group of severely hearing impaired adults performed better on tests of vowel and word perception but poorer on tests of open set sentence perception in comparison to a group of cochlear implant users.

    Speech perception in implanted children: effects of speech processing strategy and residual hearing

    The ability of implanted children to adapt to different speech processing strategies has been demonstrated for the Nucleus implant system. Children previously experienced with the Multipeak speech processing strategy. were able to gain significant improvements in consonant, word and sentence perception using the Speak speech processing strategy. suggesting some degree of neural plasticity in neural-auditory coding. Of 192 implanted children with different degrees of preoperative residual hearing, 65% were found to obtain significant scores on open-set speech materials using electrical stimulation alone. Those children with more residual hearing had a greater probability of achieving open-set understanding and at a minimum level, perceived high frequency consonant information which would not have been available through conventional hearing aids.

    Speech perception in implanted children: effects of preoperative residual hearing and speech processing strategy [Abstract]

    Since the first child was implanted with the Nucleus 22-channel cochlear prosthesis in Melbourne in 1985, the number of implanted children world-wide has rapidly expanded. Over this period, more effective paediatric assessment and management procedures have developed, allowing cochlear implants to be offered to children under the age of 2 years. In addition, a succession of improved speech processing strategies have been implemented in the Nucleus implant system, resulting in increased mean speech perception benefits for implanted adults. Research in the Melbourne and Sydney Cochlear Implant Clinics has also demonstrated that young children can adapt to and benefit from improved speech processing strategies such as the Speak strategy. Reported speech perception results for implanted children show that a considerable proportion (60%) of paediatric patients in the Melbourne and Sydney clinics are able to understand some open set speech using electrical stimulation alone. These results, and the upward trend of speech perception benefits to improve over time with advances in speech processing. have raised questions as to whether severely, or severely-to-profoundly deaf children currently using hearing aids would in fact benefit more from a cochlear implant. To investigate the potential effect of the level of preoperative residual hearing on postoperative speech perception. results for all implanted children in the Melbourne and Sydney cochlear implant programs were analysed. Results showed that as 8 group, children with higher levels of preoperative residual hearing were consistently more likely to achieve open-set speech perception benefits. Potential factors in this finding could be higher levels of ganglion cell survival or greater patterning of the auditory pathways using conventional hearing aids prior to implantation. Conversely, children with the least preoperative residual hearing were less predictable, with some children achieving open-set perception, and others showing more limited closed-set benefits to perception. For these children, it is likely that preoperative residual hearing is of less significance than other factors in outcomes.
