    A clinical report on receptive vocabulary skills in cochlear implant users

    Objective: The aim was to measure the rate of vocabulary acquisition for cochlear implant users and compare the pre- and postoperative rates with published data for other groups with normal or impaired hearing. The hypothesis was that the postoperative rate would be greater than the preoperative rate. Design: The Peabody Picture Vocabulary Test (PPVT) was administered to 32 children, adolescents, and prelinguistically deafened adults implanted with the 22-electrode cochlear implant. Age at implantation ranged from 2 y r 6 mo to 20 yr and implant use ranged from 6 mo to 7 yr 8 mo. Results: The group mean postoperative performance at various postoperative intervals was significantly higher than mean preoperative performance. Single-subject data indicated statistically significant gains over time on this test for 13 of the subjects. The mean postoperative rate of vocabulary acquisition of 1.06 times the rate for normally hearing children was significantly greater than the mean preoperative rate of 0.43. Conclusions: These rates of improvement were in accord with previous reports on smaller numbers of implant users, but could not be attributed unambiguously to use of the implant because no control group was used for this clinical work. Variables such as age at implantation, duration of profound deafness, communication mode, and speech perception skill failed to significantly predict rate of improvement on the PPVT.

    A clinical report on speech production of cochlear implant users

    Objective: The aim was to assess articulation and speech intelligibility over time in a group of cochlear implant users implanted at 8 yr or over. The hypothesis was that the postoperative speech production performance would be greater than the preoperative performance. Design: A test of intelligibility using sentences and an articulation test measuring non-imitative elicited speech were administered to 11 and 10 subjects, respectively, who were implanted with the 22-electrode cochlear implant. Nine subjects received both tests. Age at implantation ranged from 8 yr to 20 yr and implant use ranged from 1 yr to 4 yr 5 mo. Results: For both the intelligibility and articulation tests roughly half of the subjects showed significant improvements over time and group mean postoperative performance significantly exceeded preoperative performance. Improvements occurred for front, middle, and back consonants; for stops, fricatives, and glides and for voiceless and voiced consonants. Conclusions: Despite being deprived of acoustic speech information for many childhood years, roughly half of the patients assessed showed significant gains in speech intelligibility and articulation postimplantation. The lack of a control group of non-implanted patients means that we cannot separate out the influence of the implant on speech production from other influences such as training and tactile-kinaesthetic feedback.

    Potential and limitations of cochlear implants in children

    Multiple-channel cochlear implants have been in use with children and adolescents for 8 years. The speech perception, speech production, and language of many of these children has been investigated in some detail.l-4 There have been many predictions about factors that may affect the performance of children with implants. For instance, it has been suggested that children with a congenital loss of hearing would not have the same potential to benefit from a cochlear implant as those with an acquired loss. Similarly, it has been suggested that younger children are likely to gain more benefit from a cochlear implant because of the effect of various critical ages for language learning.5 As more results have become available, it has been our observation that the performance of any particular child with a cochlear implant does not appear to follow well-defined rules, and that generalizations about the potential of certain groups of children are likely to encounter many exceptions. We now have a large quantity of results for children using cochlear implants, and it may be possible to determine some of the factors that have a significant effect on performance. This paper will attempt to identify some of these factors by reviewing speech perception results for 100 children implanted with the Nucleus 22-channel cochlear prosthesis in Australia and speech perception results for adult patients. This analysis will use an "information processing" model of a child using a cochlear implant. That is, we will assume that a child will benefit from a cochlear implant in terms of speech perception, production, and language development, if he or she receives a maximal amount of auditory information from the environment, and is able to process this information successfully. This model divides potential limiting or predictive factors into those that affect the information presented to the auditory system (eg, implant technology, surviving auditory neurons) and those that affect the processing of this information (eg, development of central auditory pathways, amount and consistency of auditory input).

    Improved sound processing for cochlear implants

    Four signal processing schemes currently under development aim to improve the perception of sounds/ especially speech, for children and adults using the Nucleus cochlear implant system. The schemes are (1) fast-acting input-signal compression, (2) Adaptive Dynamic Range Optimisation (ADRO), (3) TESM, a scheme that emphasises transients in signals, and (4) DRSP, a strategy that applies different stimulation rates to selected sets of electrodes.

    Rehabilitation strategies for adult cochlear implant users

    This paper summarizes open-set speech perception results using audition alone for a large group of adult Nucleus cochlear implant users in Melbourne. The results show wide variation in performance but significant improvement over the years from 1982 to 1995. Analysis of these results shows that speech processor developments have made the major contribution to this improvement over this time. Recent results for patients using the SPECTRA-SPEAK processor show !hat most subjects obtain good speech perception within six months of implantation and the need for intensive auditory training is minimal for many of these patients. Postoperative care should encourage consistent device use by providing opportunities for success and providing long term technical support for implant users. In some cases, including elderly patients, those with long term profound deafness, and those with special needs, there will still be a need for additional rehabilitation and auditory training support.

    Factors affecting outcomes in children with cochlear implants

    Open-set speech perception tests were completed for a group of 52 children and adolescents who were long-term users of the Nucleus multiple channel cochlear prosthesis. Results showed mean scores for the group of 32.4% for open-set BKE sentences and 48.1% for phonemes in open-set monosyllabic words. Over 80% of the group performed significantly on these tas1cs. Age at implantation was identified as a significant factor affecting speech perception performance with improved scores for children implanted early. This factor was evident in the results at least down to the age of three years. Duration.. of profound hearing loss, progressive hearing loss, educational program and preoperative residual hearing were also identified as significant factors that may affect speech perception performance.

    The auditory cortex and auditory deprivation: experience with cochlear implants in the congenitally deaf [Abstract]

    The primary auditory cortex (AI) exhibits a topographic representation of the organ of Corti in normal hearing animals. Plasticity studies have shown that this orderly representation of frequency can be modified following a restricted hearing loss or by behavioural trainingl,2. Little is known, however, of the effects of a profound hearing loss on AI, although a number of early studies have suggested an enhancement of activity from other modalities3. Knowledge of the functional status of the central auditory pathway in the profoundly deaf, and the ability of these structures to undergo reorganization particularly following long periods of auditory deprivation - are important issues for the clinical management of cochlear implant patients. In this paper we review our recent clinical and experimental experience with cochlear implants in the congenitally deaf.

    Effects of rate and pulse manipulations of the spectral maxima speech processor upon speech production

    Postlingually deafened adults experience many changes in their speech as a consequence of late-onset deafness. Previous studies have indicated changes in fundamental frequency, duration, intensity, and vowel formant frequencies in postlingually deafened adults. Postlingually deafened adults who receive multichannel cochlear implants demonstrate improved control of fundamental frequency and intensity. Shifts in formant frequencies to values similar to normal hearing. speakers also appears in some adult implant users. In order to examine how adult implant users adjust their vowel production in response to map changes, we examined vowel production following manipulations to the processing strategy of the Spectral Maxima Speech Processor. This processor uses a speech processing scheme in which six spectral maxima from the outputss of 16 bandpass filters stimulate the cochlea on a place basis at a constant rate. The rate of sampling of the filterbank output is 250 Hz, so six biphasic pulses are presented every 4 msec and there is no attempt to extract fundamental frequency or to find the formant peaks in the speech signal. Two manipulations to the scheme were examined. In the first condition, the rate of sampling remained at 250Hz but eight biphasic pulses were presented rather than six. In the second condition, six biphasic pulses were presented but the rate of sampling of the filterbank output was increased to 400 Hz. Speech samples also were acquired using the standard spectral maxima processor .and with no auditory feedback when the processor was turned off. Speech samples from three �subjects were acquired immediately after receiving the manipulated speech processors and after two weeks experience with the various processors. Preliminary data indicate one subject experienced increased fundamental frequencies while using the 400 Hz high rate strategy. No significant changes were observed in fundamental frequency between the normal SMSP processor and the eight pulse variation. Elimination of feedback resulted in significantly lower fundamental frequencies. The high rate and eight pulse variations resulted in significantly higher first formant frequencies. Second formant frequencies also appear to be influenced by the processing strategies. Comparisons of values produced immediately after receiving a new strategy versus those produced after two weeks use show shifts in primarily first formant values. Data will be presented for all three subjects and discussed in regard to sensitivity to variations in speech processing schemes and the influence of experience with manipulated schemes.
