    Group Music Therapy as a Preventive Intervention for Young People at Risk: Cluster-Randomized Trial

    BACKGROUND: Music forms an important part of the lives and identities of adolescents and may have positive or negative mental health implications. Music therapy can be effective for mental disorders such as depression, but its preventive potential is unknown. OBJECTIVE: The aim of this study was to examine whether group music therapy (GMT) is an effective intervention for young people who may be at risk of developing mental health problems, as indicated via unhealthy music use. The main question was whether GMT can reduce unhealthy uses of music and increase potentials for healthy uses of music, compared to self-directed music listening (SDML). We were also interested in effects of GMT on depressive symptoms, psychosocial well-being, rumination, and reflection. METHODS: In an exploratory cluster-randomized trial in Australian schools, 100 students with self-reported unhealthy music use were invited to GMT (weekly sessions over 8 weeks) or SDML. Changes in the Healthy-Unhealthy Music Scale (HUMS) and mental health outcomes were measured over 3 months. RESULTS: Both interventions were well accepted. No effects were found between GMT and SDML (all p > 0.05); both groups tended to show small improvements over time. Younger participants benefited more from GMT, and older ones more from SDML (p = 0.018). CONCLUSIONS: GMT was associated with similar changes as SDML. Further research is needed to improve the processes of selecting participants for targeted interventions; to determine optimal dosage; and to provide more reliable evidence of effects of music-based interventions for adolescents.

    A mobile phone application for the assessment and management of youth mental health problems in primary care: a randomised controlled trial

    BACKGROUND: Over 75% of mental health problems begin in adolescence and primary care has been identified as the target setting for mental health intervention by the World Health Organisation. The mobiletype program is a mental health assessment and management mobile phone application which monitors mood, stress, coping strategies, activities, eating, sleeping, exercise patterns, and alcohol and cannabis use at least daily, and transmits this information to general practitioners (GPs) via a secure website in summary format for medical review. METHODS: We conducted a randomised controlled trial in primary care to examine the mental health benefits of the mobiletype program. Patients aged 14 to 24 years were recruited from rural and metropolitan general practices. GPs identified and referred eligible participants (those with mild or more mental health concerns) who were randomly assigned to either the intervention group (where mood, stress, and daily activities were monitored) or the attention comparison group (where only daily activities were monitored). Both groups self-monitored for 2 to 4 weeks and reviewed the monitoring data with their GP. GPs, participants, and researchers were blind to group allocation at randomisation. Participants completed pre-, post-, and 6-week post-test measures of the Depression, Anxiety, Stress Scale and an Emotional Self Awareness (ESA) Scale. RESULTS: Of the 163 participants assessed for eligibility, 118 were randomised and 114 participants were included in analyses (intervention group n = 68, comparison group n = 46). Mixed model analyses revealed a significant group by time interaction on ESA with a medium size of effect suggesting that the mobiletype program significantly increases ESA compared to an attention comparison. There was no significant group by time interaction for depression, anxiety, or stress, but a medium to large significant main effect for time for each of these mental health measures. Post-hoc analyses suggested that participation in the RCT lead to enhanced GP mental health care at pre-test and improved mental health outcomes. CONCLUSIONS: Monitoring mental health symptoms appears to increase ESA and implementing a mental health program in primary care and providing frequent reminders, clinical resources, and support to GPs substantially improved mental health outcomes for the sample as a whole.     Extrinsic Versus Intrinsic Benefits: Challenging Categories Used to Define the Value of Music in Schools

    Increasing pressures to justify the value of music in schools over recent decades has led to the construction of three distinct areas of benefits: intrinsic (or musical) benefits, extrinsic benefits related to academic and/or cognitive development, and extrinsic benefits related to psychosocial wellbeing. While some argue these categories have been useful for identifying specific areas of value and enabling targeted advocacy approaches, others have challenged this segmented approach to justification. While the most strident point of contention stems from the perception that categories which champion non-musical (extrinsic) benefits have led to the devaluing of musical (intrinsic) benefits, others dispute the categories themselves. Such arguments question the ability to separate what have been categorised as musical and non-musical elements, particularly in relation to social and political elements. This paper aims to tease out the practicality of these existing categories, and in doing so, challenge their robustness in both form and definition. The argument is made that current attempts to separate the value of school-based music into distinct categories is not only unclear, but also unhelpful in areas of advocacy. This argument rests on the premise that musical participation affords opportunities to enrich human experience in holistic and integrated ways, and that categorisation serves to preclude this unique value.

    The psychosocial benefits of school music: reviewing policy claims

    While policy-based advocacy for music in schools has previously focused on arguments for cognitive and academic benefits, scholars have increasingly critiqued this rationale. Such critique is now reflected in Australian policy documentation, which now emphasises psychosocial benefits as the most notable non-musical advantage of student music participation in mainstream schools. This paper uses recent research to review these claims, with a specific focus on the type of music participation that policy documents claim will achieve psychosocial benefits. This aims to both assess the suitability of current advocacy in this area, and bring existing evidence to the attention of policymakers. The article concludes that models of music participation must be broadened beyond the current definitions of music education if psychosocial benefits are to be experienced by students in mainstream schools.
