    Older adults' music listening preferences to support physical activity following cardiac rehabilitation

    Background: Music listening during exercise is thought to increase physiological arousal and enhance subjective experience, and may support physical activity participation among older adults with cardiac disease. However, little is known about how music preferences, or perceptions of music during exercise, inform clinical practice with this population. Objective: Identify predominant musical characteristics of preferred music selected by older adults, and explore participants' music listening experiences during walking-based exercise following cardiac rehabilitation. Methods: Twenty-seven participants aged 60 years and older (21 men, 6 women; mean age = 67.3 years) selected music to support walking over a 6-month intervention period, and participated in post-intervention interviews. In this two-phase study, we first identified predominant characteristics of participant-selected music using the Structural Model of Music Analysis. Second, we used inductive thematic analysis to explore participant experiences. Results: Predominant characteristics of participant-selected music included duple meter, consistent rhythm, major key, rounded melodic shape, legato articulation, predictable harmonies, variable volume, and episodes of tension with delayed resolution. There was no predominant tempo, with music selections ranging from slow through to medium and fast. Four themes emerged from thematic analysis of participant interviews: psycho-emotional responses, physical responses, influence on exercise behavior, and negative experiences. Conclusions: Findings are consistent with theory and research explaining influences from music listening on physiological arousal and subjective experience during exercise. Additionally, for older adults with cardiac disease, a holistic approach to music selection considering general well-being and adjustment issues, rather than just exercise performance, may improve long-term lifestyle changes and compliance with physical activity guidelines.

    A theoretical framework and therapeutic songwriting protocol to promote integration of self-concept in people with acquired neurological injuries

    A positive self-concept after neurological injury is associated with enhanced quality of life and good mental health. Therefore, effective reconstruction of identity is heralded as an important goal of rehabilitation. We have developed and tested a songwriting protocol for people with acquired brain injury and/or spinal cord injury (SCI) that focuses on six domains of self-concept (physical, personal, social, family, academic/work, and moral). Over 12 music therapy sessions, people create three songs that reflect their perception of their past, present, and future selves. The therapeutic process of creating these songs aims to integrate residual components of the past self with that of the present injured self. This article outlines the theoretical foundations for the use of songwriting as a medium for change and describes the protocol in detail. We then present a case study of a man with SCI to illustrate the application of the protocol and the ensuing changes in self-concept.

    A critical interpretive synthesis of the most commonly used self-report measures in Australian mental health research

    OBJECTIVE: To critically examine the self-report measures most commonly used in Australian mental health research in the last 10 years. METHOD: A critical interpretive synthesis was conducted using seven outcome measures that were identified as most popular in 43 studies from three mental health journals. RESULTS: Results suggest that the amount and type of language used in outcome measures is important in both increasing the accuracy of the data collected and fostering positive experiences of data collection for participants. CONCLUSIONS: Results indicate that many of the measures most often used in Australian mental health research may not align with the current contemporary philosophy of mental health clinical practice in Australia.

    Flow and meaningfulness as predictors of therapeutic outcome within songwriting interventions

    There has been inadequate consideration of how flow and meaning impact health within music therapy interventions. The purpose of these analyses was to determine if flow and meaningfulness of songwriting were related to and functioned as predictors of therapeutic outcome within songwriting interventions for adult inpatients on an acute care psychiatric unit (Study 1) and a detoxification unit (Study 2). Correlational and multiple regression analyses were conducted on data with inpatients who had participated in a single-session highly structured blues songwriting intervention with a music therapist. Therapeutic outcomes were state indices of hope (Study 1; N = 54 adults on an acute care psychiatric unit) and readiness to change (Study 2; N = 170 adults on a detoxification unit). In both studies, there tended to be positive and significant correlations between flow and meaningfulness of songwriting and therapeutic outcomes. Multiple regression analyses indicated that flow was a significant predictor of therapeutic outcome but that meaningfulness of songwriting was not a significant predictor of therapeutic outcome during both studies. Flow may represent a positively framed and less invasive method for measuring patients’ perceptions of the therapeutic outcomes. Implications, limitations, and suggestions for future research are included.

    Music-assisted relaxation during transition to non-invasive ventilation in people with motor neuron disease: A qualitative case series

    For individuals with motor neuron disease, the transition to using non-invasive ventilation has a number of associated stressors, which can negatively impact adherence to ventilation and its benefits to quality of life. Music listening interventions have been linked with a reduction in anxiety and physiological stress responses in mechanically ventilated patients. This article presents four case studies from a feasibility study that investigated the experience and effect of a music-assisted relaxation intervention on anxiety, quality of life, and non-invasive ventilation use during the first 3 months of using non-invasive ventilation. The experiences of these four participants demonstrated how music-assisted relaxation served to distract from non-invasive ventilation and promote relaxation, indicating that for some, music-assisted relaxation could be a useful strategy to optimize their non-invasive ventilation experience. Early music therapy intervention in motor neuron disease progression could be beneficial to address obstacles faced by one participant in using music-assisted relaxation.

    The Brunel Music Rating Inventory-2 is a reliable and valid instrument for older cardiac rehabilitation patients selecting music for exercise

    Music with motivating qualities might support exercise adherence in older adults with cardiac disease. The Brunel Music Rating Inventory-2 (BMRI-2), a 6-item scale with motivational quotients ranging from 6 to 42 developed to facilitate music selection for exercise, has been validated with young adults. However, the suitability of the BMRI-2 for older adults is unknown. We tested reliability (internal consistency, test-retest reliability, and measurement error) and validity (content and construct) of the BMRI-2 with older adults in cardiac rehabilitation. Eleven men and 9 women (M age = 71.6 years, SD = 8.9) selected two pieces of music and used the BMRI-2 to rate the motivational quality of these for a walking task. BMRI-2 ratings demonstrated high internal consistency. Test-retest reliability and levels of measurement error were acceptable. Content was evaluated as relevant and comprehensive. There was convergence with flow scales, and discrimination between music selections with differing motivational quotients ( p < .01). The BMRI-2 demonstrates acceptable psychometric properties for older adult cardiac rehabilitation patients.

    Reliability and Validity of the Meaningfulness of Songwriting Scale (MSS) with Adults on Acute Psychiatric and Detoxification Units

    BACKGROUND: Songwriting is an intervention with demonstrated clinical benefit for a range of clinical populations. Researchers argue that positive outcomes are in part the result of the meaningfulness of the creative process. However, no measure currently exists to quantify the extent of meaning derived from songwriting processes. OBJECTIVE: To psychometrically evaluate the Meaningfulness of Songwriting Scale (MSS) as a measure of meaning of a therapeutic songwriting process. METHOD: 147 participants receiving short-term mental health care (39 acute psychiatric care; 108 detoxification unit) were asked to complete the MSS and the Short State Flow Scale immediately following a songwriting music therapy session. Six hours later, participants completed the MSS a second time. Analyses were performed by participant cohort to determine the content validity, internal consistency, test-retest reliability, measurement error, and construct validity. RESULTS: Findings indicated that the MSS has good content validity, strong internal consistency (α = 0.98, acute psychiatric group, and α = 0.96, detoxification group), acceptable test-retest reliability (ICC2,1 = 0.93, acute psychiatric group, and ICC2,1 = 0.89, detoxification group), and construct validity (acute group was r = 0.68, p < 0.001, and detoxification group was r = 0.56, p < 0.001). Measurement error was greater in the detoxification group, suggesting that the measure may be unstable for this group. CONCLUSIONS: Preliminary evidence supports MSS use for research with inpatients on acute psychiatric units; however, cautious use is recommended for use with inpatients in detoxification units due to measurement error.
